Re: State Health Guidelines for Immunizations-2012/2013

Dear Parents,

The Park Hill School district is required to follow the state health department guidelines for immunizations. The following vaccinations must be on record at the start of the 2012-2013 school year.

Eight Grade Students- TDAP

Seventh Grade Students- Varicella (immunization and documentation of the month and the year of the disease)

In order to avoid a last minute visit to the health provider of your choice or the delay of starting school, it is recommended that students complete these requirements as soon as possible. Please bring into the health room the required documentation from your health provider as soon as it is available.
